Frequently Asked Questions

1. Do you accept transfer of care patients?
We accept most transfer of care patients. To make sure you qualify, you'll need to come to our office and sign a release of information, so we can receive records from your previous health care provider(s). Then, one of our providers reviews the records to determine if we are able to provide care for you.

2. Do you accept private insurance?
Sorry, we can't. We are an occupational health clinic and we cannot bill to private insurance companies.

3. Do you accept credit cards?
We cannot accept credit cards at this time. Please make your payment in the form of cash or checks.

4. Do you need an appointment for drug testing?
No. Walk-in drug tests are available Monday through Friday, from 8AM to 5PM. Please bring your state-issued picture ID with you.

5. Do you do CDL/ DOT physical?
Yes, we do. Please call 414-8818 to schedule your appointment for your physical.

6. How do I get my prescription refilled?
Most medications require an appointment with your physician before they can be refilled. You will need to discuss this with your health care  provider.

7. How do I get to your office?
Our office is located at 804 Allen Street Suite 1, Kelso, Washington. We are next to Crystal Glow Day Spa and 2 blocks from Three Rivers Mall. 

8. Are your forms available online to review and complete prior to appointment?
For your convenience, many of our forms are available to download here ???.

9. Do your providers see patients for general health issues?
We are only able to see injured workers with open claims.
